Reactivation of Graves’ ophthalmopathy post-thyroidectomy for Graves’ disease

Abstract Introduction The impact of thyroidectomy for Graves’ disease (GD) on ophthalmopathy (GO) is not well understood. It generally accepted that does exacerbate or precipitate GO in the majority. aim this study was to evaluate whether prophylactic glucocorticoid use following total GD prevents early post-operative reactivation GO. Methods A retrospective cohort covering 5 years, studying patients undergoing GD. Patients were divided into 3 groups. Group (Clinical Activity Score (CAS) unknown): with symptoms such as dry eyes signs inactive GO, assessed by Endocrine Surgical team; seen their local ophthalmologist, but severe enough warrant referral orbital team (tertiary centre). B (CAS=0): burnt out no team. C (CAS >0): active Orbital All given cover. Results At least one patient group (n=14), none (n=10) and five had ophthalmopathy. Post-operative worsening associated CAS >0 (p=0.0325) thyroid stimulating hormone <0.02 mIU/L (p=0.0136). Conclusion In those receiving steroid, six (17.6%) demonstrated This a TSH mIU/L, suggesting either optimisation both factors prior considering more potent steroid regimen.
